Transitional Care Center/Skilled Nursing

After hospitalization. Before you go home.

Blount Memorial's Transitional Care Center at MorningView Village provides continuity of care with skilled nursing care and therapeutic services following hospitalization. Housed in an attractive, new building that opened in 2003, the facility features 60 private rooms. Each is spacious and tastefully appointed. In-room features include cable television and telephones, as well as in-room oxygen and suction. Handicapped-equipped rooms are available for patients with special needs. The facility serves patients who no longer need acute hospital treatment but who still require medical and therapeutic services with 24-hour supervision.

The attractive public areas include four sunrooms, a spacious and airy conservatory, recreational rooms, therapy gym, music/reading room, two dining rooms and a beauty shop.

Every effort is made to ease the transition from the hospital to the Transitional Care Center, then to home,  minimizing stress and disruption to the patient and family.

An interdisciplinary team of nursing staff, therapists, case managers, registered dieticians, pharmacists and physicians provides the comprehensive treatment. Our goal is to enable patients to return home with their prior level of functional independence.

The Blount Blount Memorial Transitional Care Center at MorningView Village features varied levels of care to better meet our patients' specific needs.

Sub-Acute Care:

This comprehensive inpatient care unit is designed for individuals with an illness, injury or exacerbation of a disease process. Treatment is goal-oriented and is rendered immediately after or instead of acute hospitalization. Individuals using sub-acute care, medical and rehab services require the coordinated services of an interdisciplinary team including physicians, nurses, therapists, a dietitian, pharmacist and social worker. Our goal is helping the patient regain his or her prior level of functional independence and to safely return home. Average length of stay is less than two weeks.

Long-Term Care:

This unit provides basic medical and residential services for the elderly. Emphasis is on attaining the highest level of practical functioning. Nursing care for the medically complex also is provided.

Our state-of-the-art facility is nestled in the foothills of the Great Smoky Mountains. The Transitional Care Center at MorningView Village is at 2320 E. Lamar Alexander Pkwy. in Maryville. The location is next door to Blount Memorial's MorningView Village assisted living community and is approximately one mile south of the main Blount Memorial Hospital campus.
